Through The Valley
When one of us sees you, both of us support you. Our “Through the Valley” program ensures patients never lose access to care between discharge, in-home nursing visits, or therapy appointments.
Much like walking through a valley — where the path may feel uncertain, overwhelming, or slow — this program is designed to provide steady, connected support through the hardest parts of the journey.

How it works:
If you’re a CHC patient:
Valley Mental Health ensures continuity of provider care — offering psychiatric evaluations, therapy, and prescribing via telehealth.
If you’re a Valley Mental Health patient:
Comfort Home Care can be referred in to provide in-home psychiatric nursing, medication monitoring, and physical care when needed.
This partnership ensures no handoffs are dropped, and no care plans are stalled — so patients can keep moving forward, with confidence and support on both sides of the valley.
